Tattoo artists in Illinois are not legally able to provide services to anyone under the age of 18, regardless of consent from a parent or guardian.

Anyone with the intent to be tattooed will be required to present valid photo identification, as well as proof of inoculation against COVID-19 (including up-to-date boosters) in order to receive services.

Adults 18 and over are required to bring one (1) of the following forms of Identification to their appointment:

​

• Valid state-issued non-driver ID

• Valid driver’s license

• Valid passport

• Valid military ID (must display photo and date of birth)

Original identification documents must be physically in hand in order to be copied and filed - no photos or copies.*

*Apps, photos, or copies of vaccination card as proof of vaccination are acceptable.
